As a LPN/LVN with TheMENTOR Network, you will provide nursing services, training and support toaddress the medical and psychiatric needs of individuals with developmentaldisabilities and/or emotional or behavioral challenges.

Responsibilities:

Provide clinical care to address the medicalneeds of the individuals we serve

  • Provide prescribedmedical treatment and personal care services

  • Administer medications and provide documentationaccording to established guidelines

  • Conduct assessments, implement health care plansand respond to changes of condition
  • Ensure physiciansorders are documented and carried out as prescribed

  • Serve as liaison andadvocate with other medical and professional service providers

  • Provide input regarding health care services tobe included in each Individual Service Plan (ISP)
  • Accompany individuals to medical visits inmedical emergencies
